Location: Danbury, CT

Scope of Role:
Provide home health physical therapy services, averaging approximately 6 patient visits per day. Develop and implement individualized treatment plans, perform patient assessments, and document progress in compliance with state and agency policies.

Requirements:

• CT PT License

• Minimum 2 years of experience

Preferred:

• Home health experience
