Description
Provides strategic direction and organizational support for the intake departments of the Nuvance Health home care agencies including referral management, patient referral portals, and other related efforts. Directs the transition of patients from the hospital acute to post-acute care environment; and the transition of patients from other post-acute environments to home care, such as physician practices, SNFs and ALFs. This position collaborates with various departments, stakeholders, and operational partners in the community to develop, implement, and evaluate programs for referral management.
Directly supervises the home care intake/liaison staff.
- Directs the assessment, design, development, planning, implementation, data collection, and evaluation of the referral management process. Provides direction and leadership to the home care team regarding effective referral management.
- Directs the transition of patients from the hospital to home care; and the transition of patients from other post-acute environments to home care, such as physician practices, SNFs and ALFs.
- Participates in the SNF discharge planning meetings and patient conferences as appropriate, including onsite meetings with the SNF admissions coordinators, social workers and administrators.
- Oversees the assessment and evaluation of existing referral processes and technologies. Provides direction in program modifications based on evaluation results to keep pace with emerging trends.
- Manages/supervises the intake and liaison staff to include performance evaluations, daily feedback, and oversight of work.
- Develops a thorough knowledge base necessary to appropriately accept or refuse cases referred for care to include in-depth knowledge of home health regulations, Medicare, private insurances, Medicaid, and other payers.
- Works with the leadership of the network hospitals' care coordination departments and the physician practice care navigators to manage the at-risk patient populations.
- Participates on network-wide and community-based quality teams and steering committees as a representative of home care to improve the care of patients.
- Participates in the rotation of weekend agency supervision in the delivery of care on weekends and holidays.
- Participates in the home care agency special projects and continuous quality improvement initiatives.
- Fulfills all compliance responsibilities related to the position.

- Bachelors of Science Nursing (BSN)
- Desired:
Master's degree in public health, health administration, or nursing. - Registered Nurse
Current home health leadership experience and/or population health management; strong communication skills; ability to interface with hospital and community leaders in a professional manner; experience working with electronic medical records; flexible, organized, and detail oriented; strong knowledge of the home health referral process.
Minimum Experience5 years related experience, 2 of which must be in home health; and 3 to 5 years of supervisory/management/administrative experience.
