Description

This position is responsible for providing supportive social services to GDCHC patients and their families, including referrals and follow‑ups as required in accordance with GDCHC’s standard of care, and supporting the GDCHC providers by providing assistance with substance abuse treatment identified.

Compliance

Requires compliance with CIFC and GDCHC’s written standards, including its Compliance Program and all organizational policies and procedures (“Written Standards”). Such compliance is part of the employee’s regular performance evaluation. Failure to comply may result in disciplinary action, up to and including termination, in accordance with the CIFC Compliance Policy & Plan and the CIFC Employee Manual.

Job Responsibilities
  • Provide appropriate linkages, referrals, coordination, and follow‑up for patients to services and supports, both internally and externally.
  • Facilitate healthcare and social service system navigation.
  • Screen for social determinants of health.
  • Provide informal counseling, discuss issues related to basic needs and lack of community resources, and write progress notes based on meetings.
  • Inform, guide, and assist patients with basic needs and community resources.
  • Determine eligibility and enroll individuals into health insurance plans.
  • Educate health system providers and stakeholders about community resources.
  • Serve as point person receiving and assigning referrals for services in collaboration with Department Chief(s), including referrals from community and GDCHC departments.
  • Collect clinical data and records prior to intake appointment, including records from other facilities and required patient rating packages.
  • Communicate with other providers internally and externally regarding referral outcomes by creating provider letters, discharge letters, and summaries.
  • Participate in clinical team meetings and communicate with providers involved in patients’ care to facilitate coordination.
  • Attend provider team meetings to promote collaboration of service and treatment providers.
  • Attend and participate in individual and/or peer supervision as directed.
  • Serve as point person for tracking treatment plan updates in collaboration with providers and Department Chief(s).
  • Act as a cultural broker to provide feedback on cultural issues that may affect patients’ health, addressing health disparities and quality improvement goals.
  • Participate in outreach events in collaboration with the Outreach team to foster trust and understanding within the communities served.
  • Adhere to all HIPAA regulations and maintain confidentiality at all times.
  • Fulfill all compliance and training responsibilities related to the position, including enforcement of CIFC and GDCHC policies and procedures.

Qualifications

  • Minimum of a high school diploma or equivalent; bachelor’s degree preferred.
  • Must be from the community served or possess familiarity with the community.
  • At least 1 year of related work experience; community health experience and/or community resource knowledge strongly preferred.
  • Ability to work independently and as part of a team.
  • Professional, effective, and courteous interaction with staff and patients.
  • Experience navigating local medical and social support systems.
  • Effective verbal and written communication with diverse cultural and socio‑economic backgrounds.
  • Proficiency in MS Office Suite (Outlook, Word, Excel, etc.).
  • Reliable personal transportation.
  • Bilingual in Spanish or Portuguese preferred.
  • Knowledge of standard office policies and procedures.
  • Organizational and time‑management skills.
  • Ability to supervise work of others.
  • Strong judgment in work prioritization.
  • Ability to complete difficult or complex tasks.
  • Excellent written communication skills.
  • Ability to follow oral and written instructions.
  • Maintain strict confidentiality.
  • Positive interaction with the public.
  • Assist patients with personal information on intake forms.
  • Strong organizational skills.
  • Engage with health care providers, insurance companies, and referrals as necessary.

Current annual TB screening is required of all employees.

Physical Demands

Employee duties include sitting for long periods, speaking, writing, operating a keyboard, reading small print, viewing a computer monitor, reaching a five‑drawer filing cabinet, lifting up to 30 lbs, standing, stooping, and walking. Some evening and weekend work may be required.
