DescriptionAt Western Connecticut Home Care, we are seeking individuals to become part of our exceptional home healthcare team. This position assesses and evaluates speech, language, and/or hearing disorders. It determines and makes recommendations to the physician regarding appropriate corrective treatment. The role involves ongoing interdisciplinary assessment and development of an individualized plan of care in partnership with the patient, their representative (if any), and caregiver. The speech therapist communicates with the responsible physician and other healthcare practitioners related to the current home health plan of care.
Responsibilities- Assess the needs of the client using diagnostic speech and language evaluations to determine the type of disorder and recommend treatment to the physician.
- Participate in the development of the total care plan and case management in consultation with the physician.
- Maintain records of visits, evaluations, care plans, outcomes, responses to therapy, and discharge summaries.
- Participate in case management conferences with the nurse and team members to coordinate therapy with the overall care plan.
- Report to the physician on client reactions, condition changes, progress, and re-assess every 60 days.
- Instruct and educate family and team members on methods to assist the client in improving speech and swallowing disorders.
- Provide written instructions and demonstrations to the home health team, with copies left in the home and in the patient’s record.
- Participate in quality assurance and performance improvement plans.
- Supervise care provided by home health aides as appropriate.
- Fulfill all compliance responsibilities, including attending in-service training.
- Perform other duties as assigned.
Master's degree in Speech-Language Pathology. Must be certified by the American Speech and Hearing Association and have passed the National Registration Examination. Supervisors must be licensed in Connecticut with at least three years of clinical experience. Experience with FEES or MBS, and certifications like FIM and CPR, are preferred.

Additional InformationSalary Range: $32.64 - $65.01 Hourly. This position is exempt and classified as entry level, with employment type as other, within the healthcare industry.
