Internal Medicine - Geriatrics Physician

  Full-time, weekdays only no primary night call

• Patient Volume: patients/day

• EMR: gEHRi Med

Key Responsibilities:

• Deliver primary care to patients in post-acute care communities

• Diagnose and manage chronic and acute conditions

• Collaborate with interdisciplinary care teams, hospice, palliative, and home health services

• Ensure compliant documentation and timely billing within Medicare guidelines

• Contribute to transitions of care planning and patient-centered outcomes

Qualifications:

• MD or DO;
  Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Emergency Medicine

• Active or eligible Connecticut medical license

• Prior experience in post-acute care a plus, but not required training available
