Healthy At Home Caregivers is growing, and were hiring a Care Coordinator to support families, caregivers, and our office team in delivering non-medical in-home care.

• Manage referrals and coordinate client assessments
• Schedule and complete limited in-person assessments (designated field day typically Wednesdays)
• Communicate clearly and compassionately with families and caregivers
• Ensure assessment documentation is accurate, complete, and submitted on time
• Coordinate clean handoffs to scheduling so care starts smoothly

• Track service starting dates, renewals, and approved hours
• Follow up proactively so no services or hours are delayed or missed
• Support alignment between care delivery, documentation, and office workflows

• Maintain accurate client and caregiver records
• Handle documentation, forms, and follow-ups with attention to detail
• Support office workflows during busy periods
• Ensure actions are documented and easy for others to follow

• Support leadership with coordination and documentation
• Identify workflow gaps and improvement opportunities
• Be a reliable, calm presence in a fast-moving office

Youll be successful if:

• Assessments are completed on time and convert into active care
• Clients and families feel informed, supported, and respected
• Approved hours are not lost due to missed follow-ups
• The office runs smoothly without constant fire drills

• Ability to pass a Live Scan (background check)
• Ability to complete a TB screening
• Valid drivers license required
• Comfortable working in the office and attending limited field visits
• Use of company vehicle during work assignments
