The Case Manager is responsible for managing a caseload of beneficiaries and providing them with the guidance, support, and resources needed throughout their empowerment journey. This role focuses on helping beneficiaries overcome barriers to social inclusion, achieve financial independence, and access relevant social services. The Case Manager is expected to meet performance targets, uphold contractual obligations, and maintain high-quality service standards.

Responsibilities
  • Welcome beneficiaries and provide clear information about the program, its benefits, and impact.
  • Guide beneficiaries through the assessment process and assist with required technology (e.g., tablets).
  • Provide information about the beneficiary portal and address related inquiries.
  • Conduct motivational interviews and discuss assessment outcomes.
  • Validate post‑assessment beneficiary streams and ensure proper classification.
  • Discuss job goals, employment options, and training opportunities.
  • Direct job‑ready beneficiaries to the labor market and relevant opportunities.
  • Create individualized action plans based on available services and training options.
  • Monitor beneficiary progress and maintain accurate case notes.
  • Follow up with beneficiaries who are inactive or not progressing as planned.
  • Manage program exit procedures in line with policies and contractual requirements.
  • Collaborate with team members and contribute to continuous improvement initiatives.
Requirements
  • Minimum of 1 year of case management experience working directly with beneficiaries (required).
  • Experience in employment services (preferred).
