Are you a Certified Nursing Assistant (CNA) with Skilled Nursing Facility (SNF) experience who thrives on building relationships and making a difference in the lives of seniors?
At Gentiva Hospice, we believe every day is an opportunity to bring comfort, dignity, and peace to patients and families. We’re currently seeking a compassionate, dependable CNA to serve patients in Skilled Nursing Facilities throughout the Dallas area —while also serving as a liaison between Gentiva and facility staff to ensure quality, coordinated care.
This unique role is ideal for a CNA who enjoys hands‑on care and the opportunity to collaborate with facility teams, advocate for patients, and represent hospice services in a professional, positive way.
What You’ll Do:- Provide personal care and support to patients residing in SNFs
- Assist with daily activities including hygiene, grooming, mobility, and comfort needs
- Observe and report changes in patients’ conditions to the care team
- Build strong, trusting relationships with facility staff, residents, and families
- Support facilities in coordinating hospice services and care plans
- Represent Gentiva Hospice at facility meetings and help educate staff about hospice philosophy
- Maintain timely, accurate documentation and uphold safety standards
– What You’ll Bring :
- Active CNA license in the state of employment (Required)
- Current CPR certification (Required)
- Driver’s license and reliable vehicle with insurance (Required)
- Six (6) months or more of direct patient care experience (Preferred)
- Comfortable working in home settings, hospice facilities, or long‑term care environments
- Demonstrated compassion, patience, and ability to handle emotional situations
- Willingness to work independently and adapt to changing patient needs
- Experience in home health, hospice, palliative care, or oncology
- Prior roles in hospital nursing, ICU, geriatrics, med‑surg, ER, telemetry, or nursing homes
- Exposure to end‑of‑life care, wound care, or admissions support
