Nurse Navigator - Classical Hematology
The Nurse Navigator for Classical Hematology plays a vital role in improving referral efficiency, patient access, and care coordination for an expanding, nationally recognized tertiary referral clinic. This position supports appropriate triage of referrals, manages clinical triage communication when needed, streamlines complex care processes, and promotes consistent, high-quality patient care across the Division of Hematology and Oncology, in alignment with the stature and reputation of UT Southwestern.
Key Responsibilities- Perform timely and appropriate triage of hematology referrals to optimize access in a high-volume tertiary referral clinic.
- Improve referral efficiency by assuming triage and coordination responsibilities currently managed by physicians, APPs, and general hematology RNs.
- Obtain, review, and organize critical outside medical records, pathology specimens, imaging, and laboratory data prior to clinic visits.
- Provide clinic nursing coverage as needed, including management of triage phone calls and MyChart messages, ensuring timely, clinically appropriate responses and escalation when necessary.

This job will assist the patient, family and caregiver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.). Responsible for conducting on‑going assessments of need and facilitate referrals throughout the nursing continuum of care. Using the nursing process and evidence‑based knowledge, the Nurse Navigator provides education and resources to facilitate informed decision making (by the patient and/or family).

- Education Graduate of an approved/accredited nursing program either by a governmental authority, nursing board, or by a licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
- Experience 3 years of clinical nursing experience. Specialty areas may require 3 years of specialized nursing experience.
- Licenses and Certifications (RN) REGISTERED NURSE Licensure as Registered Nurse of Board of Nurse Examiners for State of Texas Upon Hire; (BLS) BASIC LIFE SUPPORT – current Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire
- Education Bachelor's Degree – BSN preferred
- Assess for any barriers to care through the clinical continuum to address patient, caregiver or family needs.
- Serve as point of contact for patient, caregiver, and clinical care team.
- Serve as patient advocate to assist patient and caregiver through the healthcare system.
- Provide education, resources and referrals to internal and external resources based off of assessment need.
- Facilitate shared decision making, ensuring the patient, family and caregiver are fully informed of treatment intent, treatment and outcomes.
- Promote care planning by encouraging patients to convey their wishes and goals. Work with patients to formulate questions to discuss with their healthcare team.
