DescriptionOur full-time LVN will have a variety of duties that will help the company thrive and manage continuity of care. This candidate must maintain 40 productivity points weekly to qualify and must participate in the compensated on‑call program. This LVN will see all visits assigned within the scope of practice of their license. They will also participate in the compensated perception of new hires.
Administer skilled nursing care for patients of all ages in their place of residence and coordinate care with the interdisciplinary team, patients, families, and referring agency. Assume responsibility for, and participate in, the coordination of care.
- Be available Monday‑Friday to take patients.
- Perform skilled nursing services under the supervision of a staff LVN.
- Observe, record, and report to the supervisor on the general physical and mental conditions of the patient.
- Administer prescribed medications and treatments as permitted by State regulations.
- Assist the physician and/or staff LVN in performing specialized procedures.
- Assist the patient with activities of daily living and encourage appropriate self‑care.
- Prepare clinical and/or progress notes and incorporate them into the clinical record at least weekly.
- Report to the patient's physician and other appropriate personnel about pertinent changes in the patient's condition.
- Perform complex wound care if in‑service is documented for specific procedures.
- Perform routine venipuncture to obtain labs only if documentation of competency, by a staff LVN, is in the personnel record.
- Receive and follow physician orders within the realm of practice for an LVN and within the standards of home health practice but may not alter the plan of care.
- Participate in the in‑service program, attending at least two in‑service educational meetings per year.
- Participate in nursing report.
- Participate in staff conference regarding patient care.
- Participate in QAPI plan and process.
- Active state license as a Licensed Vocational Nurse, or a multi‑state license issued by a Nurse Licensure Compact (NLC).
- 2‑3 years of experience as a Licensed Vocational Nurse.
- Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
- Proof of current CPR, TB, auto insurance, and COVID vaccine.
- Reliable transportation.
- Experience with Kinnser.
