The Social Worker Care Coordinator is a member of the Care Coordination Department who educates the healthcare team and physicians about psychosocial issues, identifies patient/family problems, and develops strategies to address them. This role applies specialized knowledge and advanced practice skills in assessment, treatment, planning, implementation and evaluation, case management, mediation, counseling, supportive counseling, direct practice, information and referral, supervision, consultation, education, research, advocacy, community organization, and administration of policies and programs.

Required – Experience and Education- Licenses and
Certifications:
Licensed Master Social Worker (LM‑SW) in the state of Texas
- At least 2 years of hospital experience
- Screen and evaluate high‑risk patients for discharge planning needs; consult with attending physicians and the healthcare team regarding identified psychosocial issues and/or care transition barriers.
- Collaborate with the multidisciplinary team to include the patient/family and assist with executing plans and interventions to facilitate the hospital stay and manage length of stay.
- Facilitate patient care conferences to establish a safe and effective discharge plan.
- Implement the transition‑of‑care plan, ensuring the patient has choices regarding agencies and services.
- Assist with abuse and neglect cases, including assessment, investigation, intervention and referral to appropriate agencies.
- Educate patients and families about community resources and interpret their needs.
- Maintain open communications with community agencies to assist in referrals.
- Maintain knowledge of payor benefits, hospital and community resources, and regulatory standards to ensure informed decision making.
- Maintain chronological notes, clinical charts, and case histories for each patient.
- Perform other duties as assigned.
