Job Description Job Purpose
  • The Care Transitions Navigator will coordinate activities that promote quality outcomes, patient throughput and discharge planning while supporting a balance of optimal care and appropriate resource utilization. The Care Transitions Navigator will identify potential barriers to patient throughput and quality outcomes minimizing delays in discharge plans.
Job Requirements Education
  • Bachelor's degree in Social Work required
  • Master's degree in Social Work preferred
Licenses and/or Certifications
  • LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferred
Work Experience
  • 1 year of experience in health related setting
  • Hospital case management experience preferred
Related Work Experience and Other Skills
  • Ability to work independently, effective time management, problem-solving skills, detail-oriented with strong organizational skills, ability to multi-task, ability to react calmly, objectively, and effectively in emergency situations, maintain a high standard of ethical behavior and personal expertise in the performance of your work.
