Position Description Essential Duties
  • Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers.
  • Provide clinical counseling as indicated by the plan of care.
  • Demonstrates appropriate psychosocial assessment skills for initial and bereavement evaluations and assists in the development and implementation of goal‑directed family care plans.
  • Actively participate in coordination of all aspects of patient’s care, in accordance with current professional standards and practice.
  • Conducts ongoing reassessments and follow‑up appraisal of patient/family situation and needs. Actively listens and responds appropriately to patient/family concerns and needs in a timely manner.
  • Assumes the active role of advocate for the patient/family.
  • Possesses adequate knowledge of community resources and discharge planning.
  • Develops and maintains positive working relationships with community agency personnel.
  • Demonstrates knowledge of the patient/family dynamics involved in death, dying, and the bereavement process. Applies knowledge of age‑specific human growth and development.
  • Demonstrates knowledge and understanding of mental and emotional dysfunction.
  • Ensures that all patients/families under his/her direct supervision are provided care and services to attain or maintain the highest practicable physical, mental and psychosocial well‑being, considering each individual patient/family.
  • Treats patient/family with compassion, adapting communication to meet situations and cross‑cultural issues.
  • Supports patient and family sense of control, while striving to reduce their anxiety.
  • Completes admission, routine visits, transfer, and discharge information as scheduled. Submits documentation on time.
  • Participates in interdisciplinary team meetings and in ongoing interdisciplinary comprehensive assessment.
  • Develops and evaluates the plan of care.
  • Contributes to patient and family counseling and education.
  • Participates in QAPI program and hospice-sponsored in‑service training.
  • Refers questions outside area of expertise to appropriate team members or community agencies.
  • Participates in Bereavement team, groups, and activities as needed.
  • Other duties as assigned by the Patient Care Manager.
Additional Information

Minimum Qualifications
  • Master’s degree in social work from an accredited college or university.
  • Licensed through the Texas State Board of Social Work Examiners.
  • At least one year previous experience working in a healthcare setting.
  • Valid Texas Driver’s License and reliable method of transportation.
  • Ability to perform essential duties as outlined below.
  • Knowledge of current professional social work theory and practice.
  • Excellent clinical assessment and documentation skills.
  • Ability to organize workflow, respond to emergencies and note detail.
  • Ability to maintain a positive, stable disposition in performing work in association with patients, families and staff.
  • Ability to maintain a high level of performance in an environment of frequent demands and changes.
  • Excellent verbal and written communication skills.
  • Genuine caring for and interest in the care of mature adults, handicapped people and hospice patients.
  • Ability to perform work tasks within the physical demand requirements as outlined below.
  • Ability to comply with the Dying Persons Rights and the employee responsibilities.
  • Ability to comply with organizational and departmental safety policies and procedures.
