Licensed Social Worker
Azza Health Care Agency
Nursing home social workers provide vital support and advocacy for elderly residents, focusing on their psychosocial needs to improve quality of life.
Key Responsibilities- Conducting assessments and developing individualized care plans in collaboration with healthcare teams.
- Advocating for residents' rights and providing emotional support to residents and their families.
- Connecting residents and families with community resources such as mental health services and rehabilitation.
- Educating staff on residents' psychosocial needs, cultural diversity, and abuse prevention.
- Facilitating group activities to promote social interaction and emotional well‑being.
- Maintaining accurate documentation of residents' progress and interventions in compliance with regulations.
Part‑time role: minimum 15 hours per week. Compensation: $33.05 – $39.80 per hour. In‑person work.
