AboutThe Role
Sterling Health Solutions is seeking a highly organized, personable, and dependable Patient Care Coordination & Scheduling Specialist to join our growing Transitional Care Management team. This role works closely alongside our Chief Operating Officer (COO) and plays a critical part in ensuring smooth patient care coordination, efficient provider scheduling, and exceptional communication across patients, clinicians, and internal leadership. Key Responsibilities- Coordinate patient care activities to ensure timely and appropriate services
- Schedule patient visits with clinicians and healthcare providers
- Manage and maintain provider calendars, adjusting schedules as needed
- Serve as a primary point of contact for patients, ensuring a positive and supportive experience
- Communicate clearly and professionally with patients, providers, and internal teams
- Support the COO with operational coordination related to patient flow and scheduling
- Maintain accurate scheduling records and documentation
- Respond promptly and professionally to patient and provider inquiries
- Strong customer service skills with a patient-first mindset
- Excellent verbal and written communication skills
- Highly organized with strong attention to detail
- Ability to multitask and prioritize in a fast-paced environment
- Comfortable working directly with executive leadership
- Professional, empathetic, reliable, and team-oriented
- Experience with Transitional Care Management, Home Health, or healthcare operations
- Experience in healthcare, patient coordination, or scheduling
- Familiarity with provider scheduling systems or electronic health records (EHRs)
