The pharmacist will provide counseling on their medication regimen and appropriate resources to educate the patient about medication instructions, uses, side effects, and additional medication therapy management considerations. The pharmacist may also participate in specialty consults such as anticoagulation counseling, heart failure counseling, outpatient parenteral antimicrobial therapy and home infusion review, solid organ transplant and left ventricular assist device (LVAD) education and pill box fill, and discharge counseling for high‑risk patients discharging from UT Southwestern Clements University Hospital or Zale‑Lipshy Rehab facility.
