Education
  • Bachelor s Degree - Bachelor of Science (BS) degree or Doctor of Pharmacy (Pharm

    D) degree from an ACPE accredited school of pharmacy
Experience
  • 1 year with experience in a hospital setting required for an inpatient position
Licenses and Certifications
  • Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required
  • Texas license with TSBP Upon Hire
  • Texas license with TSBP within 30 days of date of hire
  • ACPE-IV Sterile Compounding (IV) certification requires sterile compounding (IV) accreditation
  • Basic Life Support (BLS) certification
  • Advanced Cardiac Life Support (ACLS) certification will be required for inpatient positions within 180 days
Preferred

    1 year retail experience for retail position
Job Duties
  • Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations.
  • Performs computer order entry accurately and timely. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges.
  • Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (e.g., ADR).
  • Works independently with multi-tasking and problem solving; cross-trained and multifunctional to maintain competency in multiple areas.
  • Enforces the formulary system and follows policy when acquisition of non-formulary product is necessary. Participates in cost-effective use and selection of pharmaceuticals.
  • Communicates drug information to physicians and nurses in a courteous and professional manner. Assists in patient/family education and counseling regarding drug therapy and side effects. Documents clinical activities and cost savings clearly for follow-up by fellow pharmacists. Aims to improve patient satisfaction.
  • Manages technicians and other staff. Delegates tasks and validates medication accuracy for distribution from IV labs, central pharmacy, or automated dispensing machines. Audits and signs out all controlled substances in accordance with guidelines and policies. Investigates discrepancies and unusual usage patterns.
  • Engages in ongoing professional development through education programs, in-services, and meetings. Participates in new employee orientation and training. Takes on additional projects as needs develop.
  • Participates in the clinical program and supports P&T initiatives by monitoring target drugs for protocol changes, substitutions, IV to PO conversions, etc. Collects data and communicates opportunities for improvement. Performs therapeutic monitoring and adjusts dosages as needed based on renal function and pharmacokinetics.
  • Duties may include core functions such as direct patient care or handling patient/human-subject records.
  • Performs other duties as assigned.
