to join our Integrated Services team at our flagship campus in Dallas, TX to work collaboratively with our team of physicians.

Primary Care, Cardiology, Gastroenterology, Hospital Medicine, Neurological Surgery, OBGYN, Orthopedic Surgery, Transplant Surgery and various other specialty service lines.

Responsibilities
  • This position will act as a liaison to the Medication Therapy Management (MTM) program but will also work on transition care and other integrated care delivery needs.
  • Works in collaboration with the MTM program on outpatient medication management and utilization for MHS employees and families
  • Works closely with the ACO to help manage and develop preventative care, quality improvement and chronic disease management programs
  • Works with Hospitalist program and Primary Care physicians to develop, implement and manage transition of care management program
  • Reports directly to CMO
Qualifications
  • Board-certified Nurse Physician Assistant
  • Active Texas medical license
  • Minimum 2 years of experience in clinical practice
  • Strong interest in Medication Therapy Management
  • Ability to work effectively in a fast-paced, team-oriented environment
