Form Health is a virtual obesity medicine clinic delivering multidisciplinary evidence‑based obesity treatment through telemedicine. The clinic leverages technology and a high‑quality care team—including physicians, advanced practice professionals, and registered dietitians—to provide patient‑centered obesity management.
About the role
We are seeking an ABOM‑certified obesity medicine physician to join our multidisciplinary weight‑management team. The physician will provide direct clinical services and structured supervision for up to three advanced practice providers (APPs), enhancing the consistency, safety, and effectiveness of patient care.
Core Responsibilities- Provide direct patient care focused on evidence‑based obesity treatment and chronic weight management, collaborating with APPs to co‑manage a shared panel and not maintaining a primary patient panel.
- Review charts and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
- Offer treatment planning and medical management support for complex cases or escalated concerns.
- Prescribe non‑controlled obesity medications for patients under direct care and in coordination with APPs.
- Conduct weekly 30‑minute collaborative meetings and 30‑minute asynchronous communication with each assigned APP to review cases and provide mentorship.
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
- Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education.
- Participate in team‑based care initiatives and contribute to a culture of collaboration and high‑quality care.
- Attend relevant departmental meetings and contribute to quality improvement efforts.
- MD or DO degree, board certified and diplomate of the American Board of Obesity Medicine (ABOM).
- Minimum of 5 years as an attending physician in a primary specialty.
- Licensed to practice and prescribe legally in Texas and credentialed with major insurance payers; preference for multiple state licenses.
- Excellent interpersonal skills for patient and team interactions.
- Strong interest in mentoring, teaching, and collaborating with APPs.
- Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting.
- Adherence to evidence‑based obesity treatments, behavioral interventions, nutrition, and pharmacotherapy.
- Committed to delivering patient‑centered, culturally competent care and to the highest quality of medical care and an evidence‑based approach to obesity management.

Salary range
: $210,000 - $225,000 USD
