Ophthalmic Clinic Scribe

Retina Consultants of Texas (RCTX) is seeking an Ophthalmic Scribe to join our innovative team with the mission of “Fighting Blindness For The World To See.” The role is responsible for providing accurate documentation, summarization of the ocular examination and recommendations, ICD‑10 coding and CPT in patient charts, while maintaining patient flow and communicating appointment status at the Brenham and Cypress Clinics.

Candidates must be able to travel to various clinics to meet the needs of the patients and providers and support the clinic hours of Monday through Friday between 6:30 am and 6:30 pm. Scheduling is provided weekly and is based on the provider’s clinic, patient and surgery schedules. Clinic staff have opportunities for weekend hours if available.

Job Responsibilities
  • Accurately summarises and documents what the physician verbalises during an examination, including charting of the examination, documenting additions to the chief complaint, assessment and recommendations for treatment, justifying decisions when appropriate, and closing patient charts in a timely manner.
  • Accurate ICD‑10 and CPT coding in patient charts.
  • Observes the patient and the physician and assists when appropriate during the exam, providing assistance to the patient, additional medications/instruments, and following directions in a medical emergency.
  • Maintains awareness of the patient’s verbal and non‑verbal communication to ensure the scribe repeats information, provides written instructions, and hands out education materials.
  • Ensures the patient understands all instructions.
  • Accurately documents amendments to patient charts per practice protocols.
  • Maintains patient flow of the clinic, including confirmation of insurance authorisation prior to treatment.
  • Provides support to the clinic by helping execute Ophthalmic Technician duties as needed.
  • Alerts the physician to patient‑related forms that need completion.
  • Guides patients to the front desk after the exam and ensures proper follow‑up appointments are communicated to be made before the patient leaves.
  • Evaluates the condition of the examination room to ensure adequate supplies and safe entry/exit.
  • Notifies the physician of changes to the daily schedule, especially emergencies.
  • Facilitates communication between the physician and other staff to minimise clinic flow disruptions.
  • Performs other tasks as assigned.
Skills & Competencies
  • Knows required components of examination for appropriate CPT coding.
  • Understands Medicare and other common insurance requirements for quality reporting.
  • Training and understanding of HIPAA requirements and patient confidentiality.
  • Strong professional interpersonal skills.
  • Excellent verbal and written communication.

  • Ability to multi‑task.
  • Strong organisation with attention to detail.
  • Computer literacy.
  • Ability to spell accurately while charting.
  • Basic math skills.
  • Fluency in English; bilingual preferred.
  • Ability to educate patients regarding eye‑drop instillation and other treatment plan instructions.
  • Understanding of medical and ophthalmic terminology.
Education Requirements
  • High school diploma required; bachelor’s degree preferred.
Experience Requirements
  • At least 6 months experience in a medical office, preferably ophthalmology.
  • Scribe certification (certified ophthalmic assistant) preferred.
Physical Abilities
  • Must be able to walk and stand for the entire work shift.
  • Must be able to move frequently between standing and sitting during clinic sessions.
  • Must be able to type or write legibly in a dimly‑lit setting.
  • Must be able to hear softly spoken words.
  • Lifting required up to 30 pounds.
