Job OverviewThis is a Band 6 Nursing role based at the Flow Centre call centre.
Role and Responsibilities- Nurses will be responsible for the provision of effective, efficient and appropriate high quality nursing care within the ABUHB Flow Centre.
- The Nurse will be expected to provide high quality advice to flow navigators, facilitate intersite transfers as well as directing pre-hospital referrals to the right place at the right time.
- The nurse also requires a quality and assurance aspect including auditing & supporting the nurse in charge with service development.
- The post holder must be a Registered Nurse and is therefore bound by the Nursing and Midwifery Council (NMC) Code of Professional Conduct for Nurses, Midwives and Health Visitors.
- The ABUHB Flow Centre is an innovative service that has been set up to ensure that patients in the ABUHB area receive the best care to meet their needs. It is the first point of contact for patients accessing healthcare in the ABUHB. Primarily receiving referrals from Primary Care and the Welsh Ambulance Service Trust (WAST) along with other referring agencies.
- The post holder will be responsible for offering clinical guidance/advice on referrals to the Flow Centre.
- They will also be responsible for highlighting staffing deficits & deputise the clinical site manager as needed ensuring a safe, effective and robust service.
- In times of higher than normal service demand it is expected that the post holder will answer calls to maintain a safe and effective service. The post holder will ensure they are up to date on the most recent pathways and liaise with other professionals to ensure patients receive the most appropriate care. They will be expected to support the Clinical Site Manager to deliver an effective, safe and efficient service and will be the point of contact in their absence.
- The post holder must be prepared to support changes and improvements whilst still maintaining a fully operational service. The post holder may also be required to undertake additional training in line with the changes to the service.
- The ability to speak Welsh is desirable for this post;
