Position Overview

The Nurse Practitioner (NP) acts as part of the clinical operations team at Chen Med/Jencare, providing direct patient care in medical centers, nursing homes, skilled nursing facilities (SNF), and home settings. This role involves geriatric assessment, medical history, physical examination, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Chen Med standards.

• Assess acute and non‑ clinical problems using detailed physical examinations and patient histories.
• Analyze patient condition trends and develop, document, and implement individualized patient management plans.
• Provide comprehensive nursing care, including counseling and education for patients, families, and significant others.
• Plan patient care based on in‑depth knowledge of protocols and patient populations, anticipating physiological and psychological problems.
• Write admission, transfer, and discharge orders and manage medication and treatment orders.
• Order and interpret laboratory and diagnostic studies; refer patients for specialist consultations when indicated.
• Document progress notes and summaries in detail, participate in rounds and conferences, and communicate strategies to the multidisciplinary team.
• Recognize situations requiring immediate physician attention and initiate life‑saving procedures when necessary.
• Use advanced communication skills to solve complex situations and improve patient processes and services.
• Collaborate with physicians, nurses, PT, social workers, family, and key caregivers to transition patients to lower care levels when medically appropriate.
• Initiate adjustments and writes orders for SNF discharges, completing required paperwork.

• Collaborate with multidisciplinary team members to analyze and evaluate current health care delivery systems and implement new practice patterns.
• Participate in outside activities that enhance personal and professional growth.
• Introduce self to patients/families, explain the NP role, and facilitate conferences to review treatment goals and optimize resource utilization.
• Enhance collaborative relationships to maximize patient and family decision‑making regarding goals of care, palliative care, and hospice.
• Facilitate discharge to appropriate care levels, using preferred providers when additional services are required.
• Prescribe medication based on state practice regulations and DEA requirements.
• Perform invasive procedures independently upon completion of documented competency.
• Perform other duties as assigned and modified by the manager’s discretion.

• Current RN licensure and Nurse Practitioner certification in the state of practice required.
• Certification in Basic Cardiac Life Support required.
• Board certification by AANP or ANCC preferred and may be required for certain states.
• Current DEA number for schedule II‑V controlled substances may be required based on state of practice.
• Minimum two years of clinical experience as an NP in Family Medicine, Internal Medicine, or Geriatrics, with past autonomy in independent care decisions.
