Description:
Provides and directs the provision of home nursing care according to physician orders.
Responsible for the competent application of the nursing process.
Oversees and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Must have reliable transportation and a current driver's license.
Agency-required liability insurance is necessary.
Home visits and periodic office meetings are required.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients per day.
Different types of visits and activities have varying point values.
Expected productivity is typically around 30 points per week.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Candidates must have home health experience.
Comfortable driving to patient homes daily is essential.
Documentation must be completed within 24 hours of patient appointments.
No guaranteed hours are available with this contract.
Candidates must accept every patient scheduled, regardless of distance.
The role requires the ability to evaluate and manage a variety of patient care needs.
Patients may require care such as medication management, wound care, and assistance with activities of daily living.
Must be willing to travel up to 75 miles.
Mileage will be compensated at the IRS standard rate.
Local candidates will be considered.
Candidates must be open to on-call rotations.
