** Job Introduction
** This role is responsible for carrying out clinical triage for children and young people’s referrals into specialist services across BaNES, Swindon and Wiltshire (BSW), as well as some referrals for the School Aged Health Services in BaNES and Wiltshire. They will ensure all referrals - including urgent ones - are reviewed within the required time frames so that referrers, parents/carers, and young people receive clear outcomes, guidance, and “waiting well” support  addition, this role will work closely with the Clinical Triage Lead on multi‑disciplinary triage (MDT) for more complex or multi‑need cases, and may chair MDT meetings when delegated.

They will also support the Care Coordination team by helping respond to clinical queries and by communicating with families, education partners, third‑sector organisations, and other professionals in a timely and supportive way.

Overall, this role plays a key part in helping families navigate HCRG Care Group services and partner organisations to ensure care is joined‑up, holistic, and aligned to a stepped‑care approach.

** Main Responsibilities

** you will play a vital role in delivering safe, effective, and timely clinical assessments to ensure each young person receive the most appropriate pathways of care.
* Provide timely clinical triage for all referrals, including urgent pathways, asking informed clinical decisions based on available information.
* Support the Clinical Triage Lead with multidisciplinary triage (MDT) for complex cases and lead MDT meetings when delegated.
* Use clinical expertise to support SPA and Care Coordination administrative processes.
* Navigate referrals across HCRG Care Group and partner services to ensure stepped, holistic and joined‑up care.
* Respond to clinical queries within agreed time frames.
* Communicate complex or sensitive information to families with empathy, managing conflict and escalating to the Clinical Triage Lead when needed.
* Ensure timely communication with service users and professionals.
* Build positive working relationships with partner organisations and explain pathways and triage decisions.
* Respond to queries from other professionals.
* Adhere to organisational policies, including safeguarding, and support their implementation.
* Ensure children and young people’s voices are represented and parent/carer concerns are acknowledged in triage decisions.
