Registered Nurse, Home Health – Full Time
Located in Cumberland, MD – UPMC Western Maryland is looking for a full‑time RN to join our Home Health team. This Monday‑Friday day shift position includes rotating weekends, holidays, and on‑call coverage. Candidates with at least one year of clinical experience are highly preferred.
Job title and pay rate will be determined by education and experience. Pay rates shown are for the Professional Staff Nurse, HH/Hspc job title.
The Professional Staff Nurse is responsible for the delivery of skilled nursing services to patients in their homes according to physicians’ plans of care, agency policies, and regulatory standards. Must be able to assess and interpret patient data to determine each patient’s needs.
Responsibilities- Attend compliance training and adhere to organizational standards, policies, and procedures.
- Focus on customer service and continually strive to achieve optimal patient satisfaction.
- Assign nursing personnel (LPNs, aides) to assist in the delivery of services as ordered on plans of care.
- Supervise and teach other nursing personnel, including LPNs and Home Health aides.
- Identify and investigate organizational issues and recommend solutions.
- Identify deficits or exceptional performance issues of nursing personnel and recommend solutions.
- Facilitate change when appropriate.
- Coordinate case‑management activities within a multidisciplinary health‑care team to ensure proper delivery of services.
- Initiate, coordinate, and maintain ongoing communication with physicians and other team members.
- Attend patient‑care conferences, team meetings, and related meetings.
- Perform duties in line with UPMC Home Health Care’s service‑management philosophy toward patients, visitors, staff, peers, physicians, and other departments.
- Identify, develop, and participate in process‑improvement opportunities that enhance quality of services.
- Provide direct patient care, evaluate outcomes, and adjust nursing care as needed for optimal patient care.
- Initiate, coordinate, and evaluate patient‑ and family‑centered health teaching.
- Establish plan of care in collaboration with physician, patient, and UM Professional per physician’s orders and patient needs.
- Advise and consult with families and agency personnel as appropriate.
- Complete documentation (visit assessment, OASIS, 485, verbal orders) accurately and promptly on laptop or in written form while in patients’ homes and within agency and regulatory standards.
- Sync (transmit) data on the same day.
- Use independent judgment for emergent and non‑emergent patient‑care needs and communicate with physicians.
- Participate in on‑call coverage rotation.
- Graduate from an accredited professional nursing school (Bachelor’s degree preferred).
- UPMC Home Health Care approved certification preferred.
- Minimum of 6 months of nursing experience in an acute‑care or skilled‑care facility.
- Current licensure as a Registered Professional Nurse in the state where the facility is located or in a state covered by a licensure compact agreement.
- CPR certification required (AHA standards) within 30 days of hire.
- Current automotive insurance.
