We are in search of an experienced Care Management Nurse (Health Home) for our Cumberland, MD location. The Care Management Nurse oversees the implementation of the CCG Population Health program, designing and maintaining systems to ensure continuity of care between the OTP program and somatic/behavioral healthcare providers, and developing processes to support consumers in healthy lifestyles and illness management.

Responsibilities
  • Schedule:

    Monday–Friday, 5:30am–1:30pm
  • Act as a comprehensive care coordinator for participants, assessing needs and facilitating communication among clinicians
  • Lead practice transformation based on Population Health principles and state/accrediting body requirements
  • Champions Population Health philosophy with agency leadership and provides staff education
  • Develop and maintain relationships with primary and specialty care providers, including inpatient facilities and emergency departments
  • Monitor Population Health performance and drive improvements using data
  • Design and develop prevention and wellness initiatives
  • Establish effective use of patient registries (CRISP and eMedicaid) to manage populations and improve outcomes
  • Collaborate with specialty practices to identify data needs for care coordination
  • Monitor participants’ monthly visits to ensure appropriate Population Health care delivery
  • Enter all required data accurately
  • Perform comprehensive assessments and develop care plans for Population Health enrollees
  • Coordinate service planning and evaluate plans of care with interdisciplinary input
  • Review laboratory results and diagnostic reports with the nurse practitioner or psychiatrist
  • Identify significant clinical findings and intervene under the direction of the nurse practitioner consultant
  • Compare ongoing outcomes to baseline data to monitor clients
  • Provide client education and document appropriately
  • Support and monitor medication adherence and prescription management
  • Consult with behavioral health staff about health conditions
  • Attend treatment team meetings to plan and coordinate care
  • Assist in contacting medical providers and hospitals for care coordination
  • Support medical and nursing teams with admissions
  • Provide training on medical diseases, treatments, and medications to Health Home participants
  • Assist in training OTP staff on Population Health services
  • Provide training on medical topics to behavioral health staff as needed
  • Address observed risks to patient safety and report adverse events
  • Develop strong working relationships and maintain clear communication with team members
  • Provide dispensing coverage and related med room tasks as needed
  • Other duties as assigned
Qualifications
  • Three or more years of experience in a medical office, hospital, or similar patient-focused environment; five+ years preferred
  • Registered Nurse licensure in Maryland or eligibility under MD Board of Nursing compact provisions
  • Demonstrated teaching, coaching, and development skills
  • Experience leading complex change in a consultative role
  • Ability to motivate clinical and professional personnel
  • Strong interpersonal, verbal, written communication, and presentation skills
  • Experience educating others on a broad range of health issues
  • Ability to collaborate with a multidisciplinary treatment team
  • Team-oriented mindset with shared caseload responsibilities
  • Knowledge of Maryland healthcare industry and policy preferred
  • Ability to conduct online research for client resources
  • Knowledge of area resources and case management preferred
  • Ability to analyze and conceptualize data to achieve outcomes
  • Proficiency in Microsoft Office; ability to learn electronic medical records and health information exchange tools

The base pay for this position is up to $38 per hour. Factors such as location, experience, licensure, education, and skill level may impact pay.
