Job Summary:
The MSW Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other team members to assess, plan, implement, and evaluate proactive, complex, and safe discharge plans. They collaborate with RN Case Managers for guidance with medically complex cases and facilitate coordination of care. They serve as a resource for discharge planning and care coordination for psychosocially complex cases.
They maintain current knowledge of community resources and post-acute care options.
- Communicate effectively with people of diverse backgrounds and all levels of authority.
- Provide communication and updates to patients and guests and ensure their needs are met.
- Respond quickly and properly to irregular and emergency situations.
- Review processes, identify and recommend opportunities for improvement.
- Monitor quality of core Public Safety processes and hold subordinates accountable for customer service standards, initiating remediation when needed.
- Maintain confidentiality in all matters related to patient care and visitor issues.
- Actively participate in identifying areas of opportunity to improve the experience for patients and guests.
- Provide cultural information to facilitate support for a treatment plan.
- Ensure accurate communication of outpatient services, hospitalization details, and treatment plans to patients and families.
- May be asked to perform additional duties as assigned.
Education: Graduate Degree required – Master’s Degree in Social Work from a school accredited by the Council on Social Work Education.
Experience: 2+ years of professional social work experience or 2+ years of healthcare experience preferred. Case management or related experience preferred.
Licensure: N/A
Additional Skills / RequirementsRequired: Weekend and holiday coverage.
Preferred: Knowledge/competence of Microsoft Office products and keyboarding.
Physical DemandsPhysical Demand Code: 3A, Administration | Work Function/Activity:
Sedentary to Light Physical Demand. Requires frequent sitting, standing, walking, bending/stooping and occasional lifting/pushing/pulling up to 20 lbs. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly.
- Use of safety equipment (HEPA mask) required for infection prevention.
- On-call responsibilities as directed.
- Ability to travel between campus buildings, remote facilities, and out of town as needed.
