Overview
Come and join our dynamic Adult & Older Adult Community Mental Health Team! We are looking for dedicated individuals who are committed to transforming community mental health services for our aging population. You will be working alongside a well established MDT including, Social workers, Mental Health nurses, psychologists, pharmacists and many other AHP's who will support you in delivering excellent holistic care.
Responsibilities- Conduct practitioner-led review clinics under the supervision and guidance of the medical team, ensuring safe and effective clinical practice.
- Maintain excellent communication with service users, their families, and general practitioners. All interactions should reflect the highest standards of professionalism and clarity.
- Recognize that the team operates across a wide geographical area in East and Central Cheshire, making it a dynamic and busy environment that requires strong organizational skills.
- Develop and sustain positive working relationships with all referring agencies and members of the multi-disciplinary team. Ensure clear, consistent communication channels are established and maintained across all agencies.
- Promote service development by identifying areas for improvement within the Community Mental Health Team in the South East area and taking a proactive leadership role in implementing changes.
CWP are seeking an experienced Community Mental Health Nurse to join our team that will replace the existing consultant compliment. The post holder will cover the Crewe patch. Crewe is well known for its extensive transportation links and thriving live music scene as well as a range of amenities that make it an up-and-coming town to work in. Crewe is undergoing extensive redevelopment and investment which goes further to improve the facilities available to the public.
The post is also aligned to a thriving network of Care Community groups that support each other to tackle health inequalities, pool collective knowledge and services whilst also streamlining access to our respective areas.
Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.
