• Conduct comprehensive assessments for community-based rehabilitation.
• Develop and implement personalised treatment plans to support client independence.
• Collaborate closely with multidisciplinary teams to deliver high-quality patient care.
• Provide education and support to clients and their families to enhance treatment outcomes.
• Maintain accurate client records and documentation in compliance with best practice standards.

• Registered with the Health and Care Professions Council (HCPC).
• Minimum of 2 years’ experience in community rehabilitation.
• Must be a competent car driver, given the community nature of this role.
