Care Navigator

We are large, busy GP surgery in the Crewe area. We have an exciting opportunity for the right candidate to join our team as a Care Navigator.

Meeting and Greeting patients, arranging appointments and dealing with general administration for the practice.

We are a busy GP practice within the Crewe area. We offer a variety of patients services to our population.

Care Navigators are responsible for undertaking a wide range of reception dutiesand the provision of general support to the multidisciplinary team. Duties can include but are not limited to,greeting and directing patients, effective use of the appointment system,booking appointments, processing of information and assisting patients as required. To act as the central point ofcontact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.
