Treatment Nurse​/Wound Care Nurse

Overview

Country Villa Wellness & Rehabilitation is seeking an experienced Treatment Nurse / Wound Care Nurse (LPN) to join our Skilled Nursing Facility team. This full-time position focuses on wound care management, skin integrity, pressure ulcer prevention, and discharge planning in a long-term care setting. This role is ideal for an LPN with strong clinical skills and experience in wound treatments, documentation, and regulatory compliance.

• Perform comprehensive skin and wound assessments
• Administer wound care and treatment orders as prescribed
• Monitor wound healing progress and recommend treatment changes
• Document treatments accurately in accordance with LTC regulations
• Track and report pressure ulcers and skin alterations
• Complete Braden risk assessments and implement prevention plans
• Coordinate wound care supplies and inventory
• Educate nursing staff on wound care best practices
• Assist with discharge planning
• Collaborate with physicians, DON, and interdisciplinary team
• Participate in QAPI and regulatory compliance activities
• Maintain knowledge of current wound care standards and regulations

• Licensed Practical Nurse (LPN) - active Missouri license required
• Graduate of an approved LPN/LVN program
• Minimum 1 year wound care or treatment nurse experience in long-term care
• 2+ years Skilled Nursing Facility experience preferred
• Current BLS/CPR certification
