This position is based in Crestview, FL
, providing occupational therapy services to Covenant Home Health Care patients through our partnership with Concierge Home Care.

As a Home Health Occupational Therapist, you will deliver high-quality, individualized therapy services in patients’ homes—helping them regain independence, improve safety, and enhance overall quality of life.

• Traveling to patients’ homes to provide occupational therapy services
• Conducting comprehensive evaluations of patients’ functional abilities and home environments
• Developing and implementing personalized, goal-driven treatment plans
• Educating patients and caregivers to promote independence and successful outcomes
• Collaborating with patients, caregivers, Covenant leadership, and the Concierge clinical team
• Supervising and delegating care to Certified Occupational Therapy Assistants (COTAs), as applicable
• Completing timely, accurate documentation using streamlined tools designed to maximize patient-facing time

• Unencumbered Florida Occupational Therapy license (required)
• Valid driver’s license, reliable transportation, and active auto insurance (required)
• CPR certification (preferred)
• Ability to supervise and delegate to Certified Occupational Therapy Assistants (preferred)
• Home health experience (preferred, but not required)
