Description

The Department of Health and Human Services, Behavioral Health Branch, is seeking a qualified candidate for the position of Deputy Public Guardian/Conservator. This position is classified as full-time.

Under direct to limited supervision, performs a variety of case coordination duties involving the Office of the Public Guardian’s case load. Performs routine investigations for possible conservatorship cases; prepares recommendations on the need for conservatorship; provides conservatorship services for persons appointed to the Public Guardian by the Courts or as required by law; and performs related duties as required. Supervision Received and Exercised:
Employees within this classification receive general to limited supervision within a framework of policies and procedures. Receives direct supervision from the Appointing Authority or designee as assigned;
May provide supervision over program staff as assigned.

Detailed Work Activities

Essential duties may include any of the following tasks, knowledge, skills and other characteristics. The list that follows is not intended as a comprehensive list; it is intended to provide a representative summary of the major duties and responsibilities. Incumbent(s) may not be required to perform all duties listed, and may be required to perform additional, position-specific tasks.

  • Responsible for overseeing and managing a caseload of individuals who are being served by the Public Guardian’s office.
  • Provides legal supervision over wards and conservatees as specified in the Probate Code and Welfare and Institutions Code.
  • Evaluates individuals in a variety of settings, including field situations and residential facilities, for development of service plans and serves as plan coordinator, when required.
  • Develops case plans, including determination of living arrangements or suitable placements in residence or care facility; monitors implementation of case plans; arranges medical treatment and personal care of conservatees.
  • Participates in case conferences and consults with treatment and/or support teams to review cases and recommendations; implements case plan, when developed.
  • Prepares detailed patient record documentation and maintains case records, including intake and referral plans, progress notes, and assessments.
  • Serves as the coordinator of care to ensure appropriate services are wrapped around the client and utilizing resources succeed, while ensuring the health and safety of client.
  • Makes periodic visits to evaluate and monitor the delivery and quality of care given by treatment providers, determine special needs, and perform personal services.
  • Recognizes and analyzes problems, gathers relevant information, establishes facts and draws conclusions.
  • Serves as a member of the quality improvement/multidisciplinary team providing critical input relating to client diagnosis and recommended service plans.
  • Reviews and analyzes the background of individuals to determine the need for a conservator; discusses case with physicians, mental health workers, probation, attorneys and other social services agencies.
  • Interviews client, family, friends and/or neighbors to gather information on the client’s needs when assessing needs for conservatorship.
