FPN Northside Family Medicine, 308 West Market Street, Crawfordsville, Indiana 47933. Franciscan Physician Network is seeking a full-time Family Medicine Nurse Practitioner to help meet the growing demand for primary care services at the Northside Family & Internal Medicine Crawfordsville location in Crawfordsville, Indiana.

WHO WE ARE LOOKING FOR:

Ideal candidate will have experience treating all ages, a focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physicians to coordinate and deliver the highest quality of patient care.

• Flexible provider schedule;
  No weekends or holidays
• Average outpatient volume: 18 - 20 patients per day
• Will treat patients of all ages
• Telehealth offered

  • Child and adult wellness visits
  • Chronic Back pain
  • Diabetes
  • Hypertension
  • Heart failure
  • Physicals
• Established and busy practice
• Strong mentorship from physicians in the practice

• Master s Degree - Required
• Nurse Practitioner (NP) or Nationally Recognized Certification - Required
