In-Home Occupational Therapist; OT — Patient-Focused Care

Overview

Deliver in-home patient care based on individualized care plans. Assess and address patient care needs, including monitoring for changes in condition and providing appropriate support for daily living and health management. Administer medications, including insulin, and perform diagnostic tests (e.g., blood pressure, glucose, wound assessments). Monitor patient progress, address caregiver concerns, and update physicians and Blue Skies Nursing supervisors as needed.

Educate caregivers and families about care routines, aftercare, and health management. Collaborate with healthcare professionals to optimize care plans and provide recommendations for enhanced healthcare.

Qualifications:

Active RN license, CPR certification

Experience:

Previous experience in a similar role or relevant training.

Skills:

Strong assessment, problem-solving, and communication skills.

Traits:
Compassionate, resilient, and adaptable to varying patient needs.

Requirements:
Reliable transportation, auto insurance and a valid driver’s license.

Flexibility:
Are able to at least 10 hours per week and work some nights and weekends.

Description:

Blue Skies Nursing is looking for part-time and full-time CNAs to provide high-quality, one-on-one home nursing care for our clients in the comfort of their homes.

Key Responsibilities:

• Support clients with ADL’s (Activities of Daily Living) including bathing, dressing, and feeding.
• Assist with patient mobility
• Provide emotional support, ensuring the comfort and dignity of each client.
• Promote quality of life by encouraging patients to engage in hobbies, interests, and activities that bring them joy and foster a sense of purpose.
• Monitor patient progress, address caregiver concerns, and update RNs and Blue Skies Nursing Supervisors based on the scope of practice.
