Licensed Practical Nurse (LPN) – ACT Team Nurse

Join Clarvida as an ACT (Assertive Community Treatment) nurse and become a critical member of a multidisciplinary team caring for adults with severe mental illness. You will provide medical treatment and support services in clients’ homes, community settings, and the office.

• Meet clients in their homes, community, and office (including Zoom/phone).
• Perform basic vitals and labs.
• Administer long‑acting injectable medications.
• Provide medication‑management support and coordinate care with medical providers.
• Transport clients using the company car as needed.
• Participate in a one‑week on‑call rotation every 7‑10 weeks. On‑call may be handled virtually or in person.

• LPN license in Louisiana.
• Experience treating adults with severe and persistent mental illness and/or substance‑abuse issues.
• Psychiatric nursing experience preferred.
• Valid driver’s license in good standing and current auto insurance.

• Hourly pay: $23.07–$24.50.
• Standard weekday schedule (8 a.m.–5 p.m.) with flexible hours.
