Care Management Operations Manager; NP

Puzzle Healthcare is hiring a Care Management Operations Manager!

Must reside in the Greater Atlanta area. Local travel required.

The NP/PA – Readmission Prevention Operations role at Puzzle Healthcare is a care-management–focused position designed to drive consistent adoption and execution of Puzzle’s Readmission Prevention Program across skilled nursing facilities (SNFs). While this role includes limited clinical rounding, its primary purpose is provider and facility education, program enablement, and interdisciplinary engagement—not acute clinical coverage. This individual functions as a clinical educator and program leader, ensuring that Puzzle providers and facility teams understand, adopt, and consistently apply readmission prevention workflows.

Education and training may be delivered virtually or in person based on operational needs.

• Perform limited rounding within assigned skilled nursing facilities to maintain clinical context and program alignment
• Observe care delivery workflows to inform education and program optimization
• Maintain clinical credibility with providers and facility teams through active engagement

Note:

This role does not provide acute change-of-condition coverage, on-call support, or escalation response.

• Lead facility-level education on Puzzle’s Readmission Prevention Program
• Conduct provider education sessions related to care management workflows
• Own new provider training centered on care management and readmission prevention
• Deliver education virtually and in person as needed

• Drive program awareness across nursing, therapy, social work, MDS, and administration teams
• Serve as liaison between providers, care management leadership, and facilities

• Track education completion and engagement metrics
• Provide feedback to leadership on program barriers and optimization opportunities
