Coventry Intergrated Palliative Care Team supports patients, families and carers with specialist palliative care needs, palliative support, and end of life care within the community setting. The post holder will be required to work as part of the integrated night palliative care multidisciplinary team and will work with other health care professionals to provide specialist advice, support, and high-quality patient centred care to those affected by malignant and non-malignant disease at the end of life and generalist community patients that require urgent interventions overnight within a 2-hour rapid response.
This will be provided 7 days a week 19:30-08:00 on a shift rota basis.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. To provide out of hours palliative and generalist unplanned community care over night, providing direct assessment and advice for patients with cancer or an advanced terminal illness and their families, who require specialist palliative care or generalist urgent care.
To Be responsible for the coordination and provision of unplanned care to patients and their careers as part of a 24-hour nursing service. To act as a specialist resource for community and primary care colleagues in the management of patients with specialist palliative care needs and urgent generalised nursing needs. Actively manage unscheduled visits to patients in a 2-hour rapid response timely manner.
