A Cardiothoracic SCP is a high profile, accessible and responsive leader, with managerial and clinical responsibility for the delivery of advanced practice in Cardiothoracic surgery. The post holder will demonstrate clinical assessment, critical thinking in the clinical decision‑making process. They will work collaboratively with the Cardiothoracic surgical team to meet the needs of patients and will play a key role in service delivery and adherence to policy and procedures.

The Surgical Care Practitioner (SCP) is required to be an experienced registered healthcare professional working at an advanced level in clinical practice, who will exercise autonomy and decision making in a context of complexity, uncertainty and varying levels of risk, holding accountability for decisions made.

Responsibilities
  • Co‑ordinate the care of patients throughout their pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.
  • Accountable for own actions in accordance with their professional regulatory body requirements.
  • Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
  • Lead and deliver clinical care across a variety of settings, according to national and local guidelines ensuring delivery of evidence‑based care and working towards the Trust vision of providing a World Class Service.
  • Using specialist knowledge assess patients donor sites for conduit harvest such as Long Saphenous vein and radial artery, taking into account varicosities, collateral supply and graft calibre with regard to their use in coronary artery bypass surgery, accurately documenting findings in patients notes and communicating findings to consultant surgeon.
  • Establish invasive and non‑invasive monitoring as required, under medical guidance.
  • Lead advanced life support when required, performing the resuscitation of arrested or compromised patients. Assist in emergency chest openings in conjunction with medical staff in line with the CALS protocol.
  • Accurately document the operative procedure in the patient’s notes following surgery, with regard to surgical sites, problems in theatre and the stability of the patient returning to the ICU.
  • Input accurate information onto trust databases in a timely manner ensuring patient documentation is kept up to date.
  • Under medical instruction, administer local anaesthetic to patient’s wounds peri‑operatively for post‑operative pain relief or pre‑operatively for minor procedures.
  • Undertake pre‑operative assessment of patients, including detailed history taking and cardiac, respiratory and abdominal assessments. Accurately document findings and discuss any findings with the supervising clinician.
  • Following completing the recognised training course. Safely obtain or reconfirm informed consent from patients for surgical procedures and blood transfusion. Following trust guidelines and assessments completed from each consultant for each procedure.
  • Act autonomously to assess, prioritise and provide expert clinical care which contributes to the patients’ treatment plan to ensure that optimum physical, psychological, and social care needs are met and ensures quality care.
  • Request and interpret relevant investigations to facilitate the development of the patient’s management plan.
  • Initiate the management of patients who are critically ill or deteriorating within scope of practice.
  • Proactively refer to other clinicians (own or other services) depending upon patients’ needs to deliver care beyond your limitations and scope of practice.
  • Proactively initiate treatment plans for patients requiring intervention and treatment ensuring ethically based considerations.
