We are recruiting for an experienced pharmacy technician looking to join a dynamic, forward thinking primary care network with an established team of clinical pharmacists and pharmacy technicians.

This role will be focused on working with practice and PCN teams to promote safe and effective prescribing and medicines management.

Full time or part time hours available. This role will involve liaising with Primary and Secondary Care services.

Main duties of the job

Our clinical pharmacy team is developing and we are looking for a motivated individual who is flexible to respond to the developing nature of the role and the needs and priorities of individuals requiring support.

You will work with our team of Clinical Pharmacists, GP Practices and care coordinators to support the safe, accurate and timely prescribing of medication to patients.

This will include monthly checks to ensure all blood monitoring up-to-date, liaison with GPs, hospital teams, PCN pharmacists and community pharmacy to ensure the safe, effective prescribing of medication.

Sowe Valley Primary Care Network is a well established collaboration between ten GP practices covering the East of Coventry with 84,000 registered patients.

The Primary Care Network has developed a rich multi-disciplinary team to support it's member practices, improve access and enhance the range of services available to patients including:

  • Nurse associates
  • Social prescribers
  • GP assistants
  • Podiatrist

We have close links with the Coventry and Warwickshire Training Hub and local universities with a wide range of training and professional development courses available to develop our team.

Job responsibilities

Key duties will include:

  • Management of DMARD monitoring for patients
  • Shared are medication
  • DOACS monmtioring
  • Liaising with Primary & Secondary Care
  • Responding to Safety Alerts
  • Medication reviews
  • Audits, guidance and prescribing reports
  • Respond to medication queries from patients and carers including consultations
  • Provide medicines guidance and support to the primary care team and community staff
Person Specification Qualifications
  • Pharmacy technician registered with GPhC
Experience
  • Experience of working with medicines reconciliation
  • Experience of working in primary care
  • Experience of working with EMIS and Docman
Disclosure and Barring Service Check
