The CINSS team is a community based interdisciplinary therapy team; supporting the early discharge of patients post discharge from hospital with a neurological diagnosis. Based at Newfield House providing patient centered rehabilitation to patients in the Coventry area aged 18 years and over. You will be an integral part of a friendly and supportive team working with people in their own homes, our out-patient clinic and gym.
This post offers the successful candidate opportunity to consolidate current skills working closely with the Clinical Lead, Service Manager and Therapy Leadership Team to deliver standards of care and service delivery.
This post will provide you with an opportunity to develop clinical skills managing patients' rehabilitation needs, develop interdisciplinary team working, and management of a team of therapy staff.
This post would suit an experienced Neuro Occupational Therapist who is looking to work within a dynamic and friendly team who are committed to providing an excellent patient-centered service, whilst supporting continuous development of clinical skills through joint working, access to a comprehensive in-service training programme.
The successful candidate will have evidence of post-graduate neuro rehabilitation experience and sound clinical reasoning. There is expectation that the candidate will be able to evidence specialist clinical experience/practice and be committed to providing high standards of care.
Main duties of the jobAn inclusive team approach and excellent communication skills are essential. You will be expected to work independently and effectively with a secondary care stroke team to ensure safe and effective discharge into the community, prioritising your workload and time, and supporting others to do the same.
- To liaise closely with healthcare professionals from UHCW and Warwickshire, Integrated and Community Care Services.
- To follow defined stroke and neurological care pathways and other relevant rehabilitation pathways.
- To assess for and plan, implement and evaluate outcome focused interventions/treatment.
- To provide a comprehensive range of evidenced based assessment and treatment to patients.
- To support the Clinical Lead and Service Lead with professional leadership within the team.
- To support the Service Lead with day-to-day operational leadership of the team in conjunction with Lead therapists in the team.
The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.
