Days vary — 10:00 AM–6:00 PM, Every Other Sat/Sun 10:00 AM–6:00 PM
- Position Type:
Full-time, non-exempt, hourly supervisor role. - Scope of Role:
Supervises a single program or unit within a program, typically supporting 4–8 individuals in residential settings or 10–15 individuals in periodic, vocational or day programs. Duties include direct support, professional or program activities, and supervision. - Key Responsibilities:
- Supervise Direct Support Professionals
- Oversee day-to-day operation of the program including delivery of supports, implementation of person-centered plans, health care, advocacy, customer engagement, regulatory compliance and, when applicable, oversight of the physical environment.
- Generally works 8 to 16 flexible hours per week in a supervisory capacity, and provides direct support for the remaining hours.
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.
Services And Supports- Direct Support:
Work regularly scheduled shifts as a direct support professional. - Records:
Documents as required; reviews individual records, billing and other program documentation for accuracy and completion daily. - Program Planning:
Assist the Program Director in developing individual program plans; train and monitor implementation of plans daily across all shifts. - Team Meetings:
Attend and assist with routine progress meetings. - Compliance:
Assist the Program Director in maintaining licensing compliance; implement Sevita compliance plan; manage individual rights implementation. - Management of Individuals’ Assets:
When assigned, ensure safety of individuals’ funds and property; monitor spending and reconcile monthly. - Incidents:
Complete or assist DSPs in documenting incident reports; submit initial incident reports to the Program Director. - Billing and Utilization:
Compile census or attendance information; review data for accuracy; submit data to the Program Director or appropriate billing authority.
- General Health Care:
Monitor health of persons served; document; communicate with medical professionals as appropriate; implement preventive strategies to limit illness and serious incidents; review documentation across all shifts. - Appointments:
Schedule health care appointments when applicable; may accompany individuals; assist with communication; document visits; oversee follow-through of orders; communicate results with families, guardians, case managers, day programs or others. - Medications:
When applicable, administer or support self-administration of medications and document delivery; review medication documentation for accuracy; communicate changes and provide training; maintain security of medications and supplies. - Medical Supplies and Equipment:
Purchase and maintain supplies and equipment; communicate with staff; assist with training on proper use. - Dietary planning:
Develop menus based on diets and preferences, purchase or oversee purchasing of food and supplies as needed, and supervise food service in line with individual plans.
- Communications:
Maintain productive and professional relations with individuals, families, case managers and other stakeholders. - Customer Service:
Implement Sevita Customer Service Standards; train DSPs in delivery.
- Recruitment and Hiring:
Assist with recruitment; interview and hire DSPs in consultation with Area Director and Talent Acquisitions. - Employee Scheduling:
Maintain schedules to cover all shifts and minimize overtime. - Training:
Assist with onboarding; deliver orientation and ongoing training; verify training records are accessible and completed. - Evaluation:
