We are a leading provider of innovative medical care for long-term and post-acute patients across the U.S., utilizing advanced technology to proactively manage patient health. Seeking a full‑time on‑site Nurse Practitioner or Physician Assistant in Poverty, TX, to deliver compassionate geriatric care in Skilled Nursing Facilities. No weekends, nights, calls, or holidays required.
Responsibilities- Conduct assessments, develop care plans, manage chronic and acute conditions, order diagnostics, prescribe treatments, educate residents and families, and collaborate with the healthcare team.
- Maintain accurate documentation and participate in quality improvement initiatives.
- Current Texas license and board certification as a Nurse Practitioner.
- Geriatric care experience preferred.
- Strong assessment and communication skills.
- Proficiency with EMR systems.
- Ability to travel occasionally.
