LVN Home Health and Hospice

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Home Health & Hospice team. In this role, you’ll provide essential care and support to patients in their homes during various stages of recovery or end-of-life care. • Deliver direct nursing care in the home setting under the supervision of a Registered Nurse
• Monitor patient conditions and report changes to the clinical team promptly
• Administer medications, treatments, and prescribed therapies
• Educate patients and caregivers on care plans, medication management, and symptom control
• Maintain thorough and accurate documentation in compliance with agency and regulatory standards
• Participate in the on-call rotation, including evenings, weekends, and holidays as needed

• Current and active LVN license in the State of Texas
• Current CPR certification (BLS required)
• Minimum of 1 year of clinical nursing experience required; home health or hospice experience preferred
• Bilingual (English/Spanish) preferred
• Strong communication and critical thinking skills
• Must have reliable transportation, valid driver’s license, and insurance (position involves regular travel to patient homes)
