We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Home Health and Hospice team. In this role, you'll provide essential care and support to patients in their homes during various stages of recovery or end-of-life care. This is more than a job; it's a calling to make a lasting impact on the lives of patients and their families.

• Deliver direct nursing care in the home setting under the supervision of a Registered Nurse
• Monitor patient conditions and report changes to the clinical team promptly
• Administer medications, treatments, and prescribed therapies
• Educate patients and caregivers on care plans, medication management, and symptom control
• Maintain thorough and accurate documentation in compliance with agency and regulatory standards
• Participate in the on‑call rotation, including evenings, weekends, and holidays as needed

• Current and active LVN license in the State of Texas
• Current CPR certification (BLS required)
• Minimum of 1 year of clinical nursing experience required; home health or hospice experience preferred
• Bilingual (English/Spanish) preferred
• Strong communication and critical thinking skills
• Must have reliable transportation, valid driver's license, and insurance (position involves regular travel to patient homes)
