Your goal as a mental health professional is to create discharge plans that address patients’ psychological, social, cultural, and financial needs, helping them recover and maintain overall well‑being. This role acts as a mediator between clinical teams, patients, families, and other care providers to ensure seamless, comprehensive care.

• Conduct individual, group, and family psycho educational groups.
• Contribute to treatment plan development and attend treatment planning meetings and reviews on an ongoing basis.
• Assess environment and patient safety status and report or respond to actual and potential safety issues or changes in the patient’s condition.
• Identify risk factors, screening criteria, and interventions for patients with special precautions such as suicide, homicide, falls, or elopement.
• Complete relevant assessments, group notes and all other documentation promptly and in compliance with requirements.
• Collaborate with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
• Conduct post‑discharge follow‑up as indicated.

• Nonviolent Crisis Intervention certification must be obtained within 30 days of employment start date.
• Certifications as an Addictive Counselor (AC).
