Overview
Job Summary:
As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Demonstrates performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization. Remains flexible to changing systems; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats co-workers, patients, families, and all members of the health care team with dignity and respect.
- The Registered Nurse demonstrates proficiency by exhibiting the following skills, competencies, and behaviors:
- Leadership:
- Upholds Kaiser Permanente policies and procedures, principles of responsibilities, and applicable state, federal and local laws.
- Serves as a leader of the health care team; delegates tasks appropriately, and demonstrates accountability.

- Utilizes communication strategies including chain of command and issue escalation to achieve outcomes.

- Demonstrates ability to problem solve with other departments to assist member problem resolution.
- Prioritizes, delegates, and supervises work assignments to ensure completion of patient care activities.
- Complies with regulatory requirements, policies, procedures, and standards of practice.
- Nursing Process:
- Develops and contributes to an individualized plan of care reflecting assessment, planning, implementing, and evaluating outcomes.
- Ensures plan shows multidisciplinary planning, consultation, and education.
- Ensures plan reflects admission or outpatient data, ongoing findings, age-appropriate care, cultural needs, and appropriate acuity.
- Ensures plan is discussed with patient, family/significant others, and completed in a timely manner.
- Monitors patient progress based on the plan; revises the plan as needed based on progress and condition.
- Ensures care meets standards of practice and supports discharge planning including patient education.
- Ensures patient safety related to medications and procedures, including five rights, falls prevention, decubitus prevention, and infection prevention.
- Ensures optimal pain control and patient comfort; addresses patient anxieties, fears, or concerns regarding condition, treatment, or discharge.
- Ensures patient understands medication purpose, side effects, and administration instructions at hospital discharge.
- Patient Care
Experience: - Practices customer service standards as defined by the Service Area, Medical Center, and department.
- Promptly answers call lights, alarms, and patient requests; makes appropriate referrals and facilitates access to resources.
- Maintains and protects patient confidentiality; ensures a clean, orderly, and functional work environment; treats families with courtesy, respect, kindness, and compassion.
- Provides an optimal patient care experience by listening to needs and ensuring comprehension of information provided.
- Delivers care that meets or exceeds patient expectations.
- Team Commitment:
- Is viewed as an effective team member; remains flexible, cooperative, and willing to assist others.
- Constructively handles conflict and seeks appropriate assistance when needed.
- Takes accountability for actions and accepts constructive feedback; acts as a resource, preceptor, and mentor to new staff and students.
- Attends mandatory meetings and participates in professional development; supports unit learning needs and annual education planning.
- Keeps informed about unit activities and contributes to change as needed.
- Adheres to Attendance Program and reports promptly to assigned area.
- Supports a collaborative Labor-Management Partnership through unit-based teams.
- Fiscal Responsibility:
- Organizes work to minimize overtime and identifies systems improvements; utilizes payroll and non-payroll resources effectively.
Basic Qualifications:
- Minimum one (1) year of recent full-time equivalent experience in a licensed Home Health, Palliative Care and/or Hospice agency as an RN/PHN.

- Registered Nurse License (California)
- Driver's License (California)
- Basic Life Support
- Clean driving record for the last 2 years.
- Proof of automobile insurance as required by law.

- If ten (10) hour option chosen; shift will be 8:30 am to 7:00 pm. Back up 8-hour shift will be 8:30 am to 5:00 pm.
- Weekends on rotation.
