The RN Complex Case Management and Readmission Coordinator is responsible for the comprehensive case management of medically and socially complex inpatients. This role focuses on reducing avoidable readmissions by identifying high‑risk patients, developing individualized care plans, coordinating multidisciplinary resources, and ensuring safe and effective transitions of care. The RN collaborates closely with physicians, nursing staff, social workers, and community partners to promote optimal patient outcomes.

Collaborate with multidisciplinary teams to create and implement individualized care plans for high‑risk patients, coordinate resources, monitor progress, and ensure smooth transitions of care to minimize readmission rates.

Since its founding in 1965 as Corona Community Hospital, the institution has evolved into a 238‑bed community hospital network, comprising a 160‑bed acute care hospital and a 78‑bed rehabilitation campus. The hospital is certified by The Joint Commission and employs more than 1,250 trained healthcare workers with a medical staff of approximately 347 physicians across more than 40 specialties.

• Associate’s or bachelor’s degree in nursing (BSN preferred).
• Current Registered Nurse (RN) license in the state of practice.
• Case Management Certification (ACM, CCM, or equivalent) preferred.

• Minimum 3–5 years clinical nursing experience, preferably in acute care.
• Two+ years in case management, care coordination, or discharge planning.
• Experience managing complex patient populations and reducing readmissions is strongly preferred.

• Strong critical thinking and problem‑solving skills.
• Excellent communication and interpersonal abilities.
• Proficiency in EMR and case management systems.
• Knowledge of community resources, transitional care models, and value‑based care initiatives.

Salary offers may be based on key factors such as education and related experience. Starting from $48.33 to $72.49 an hour.
