Under the leadership of the Administrator, Nursing Home, the HIM Clerk is an active member of the nursing home team that delivers professional HIM services and support that is consistent with the strategic vision, goals, philosophy, and direction of HIM and CRHS. The HIM Clerk prepares new patients’ charts being requested for admission to the nursing home. The Clerk retrieves, logs, and delivers medical records daily as assigned to all nursing floors;

emergency room department as requested. The Clerk is responsible for retrieval, assembly, and analysis of all discharge medical records for completeness and deficiencies and ensures deficiencies are corrected.

Basic Qualifications:

  • Education: Requires a high school diploma or a GED state certification.
  • Experience: Requires up to three months of work-related experience or any equivalent combination of education, training, and experience.

    This job does not require any professional or technical licenses, registrations, or certifications.
  • Upload all admission materials and prepare initial patients’ charts for admission to nursing home.
  • Retrieves all medical records requests and processes as indicated
  • Utilizes the electronic health record, health information systems, and healthcare applications (Point Click Care).
  • Responsible for scanning all documents and uploads to the EMR.
  • Attend morning meeting and report on all transportations for patients, destinations and cost to be covered by Medicaid.
  • Reviews discharge records for completion, identifies deficiencies and works to rectify them.
  • Monitors physician visits and keep visit dates up to date in PCC and on the spreadsheet
  • Collaborating with internal teams to address any delays or issues with document retrieval
  • Classifies, sorts, and files records.
  • Performs general office operations, maintains files, and retains nursing home records.
  • Faxes and/or mails out records to external reviewers.
  • Arrange transportation for residents as deemed appropriate.
  • Coordinate the Driver schedule daily to ensure efficient and timely transport to appointments.
  • Serve as a backup Driver if needed to assist in transporting patients.
  • Attend the QAPI meeting monthly and present appointment transportation
  • Attends in-service training, education programs and meetings as required or directed.
  • Performs other related job duties as assigned
