Position: DIRECT SUPPORT PROFESSIONAL

Responsibilities
  • Provides close supervision for individuals in accordance with their individual treatment plan; if the individual is on a 1:1 supervision status, supplies continual supervision as noted in the plan and completes the transfer form when transferring the individual's care to another staff member.
  • Participates in the individuals’ program planning meetings and reviews and signs off on Annual and Semi‑Annual Treatment Plans.
  • Maintains individuals’ possessions in a neat and orderly manner, communicates their needs to management, and completes documentation in accordance with program procedures.
  • Transports individuals safely and comfortably, follows all NYS laws and agency procedures, maintains a valid NYS Driver’s License, completes all documentation (including vehicle logs) promptly, and ensures the vehicle’s conditions meet the individuals’ needs (e.g., appropriate temperature and radio volume).
  • Maintains the physical plant and agency property in a clean and safe manner and reports any issues to management.
  • Provides all other duties as needed or requested to ensure that individuals are safe, healthy, and comfortable.
  • Complies with OPWDD regulations and agency procedures.
  • Obtains and maintains SCIP‑R certification and attends all mandatory training.
  • Obtains and maintains AMAP certification and submits a copy of the certification to each program in which the staff administers medication.
  • Attends all staff meetings, including in‑services and trainings.
  • Demonstrates competency in all required training areas.
Travel Required

Yes. 50% of the time.

Qualifications
  • A minimum of a high school diploma or GED.
  • A clean and valid NYS driver’s license.
  • Experience working with people who have developmental disabilities or traumatic brain injury (preferred).
  • Ability to complete SCIP and AMAP training.
Education

High School or better.

Licenses & Certifications

Obtains and maintains SCIP‑R certification; attends all mandatory training. Obtains and maintains AMAP certification and submits a copy of the certification to each program in which he/she administers medication.
